Our mobile clinical charting technology – Point of Care – helps hands-on clinical staff more accurately document resident care and promotes timely communication between clinical staff, physicians, etc. POC combines the convenience of portable device documentation, the integration of new data with existing records and the virtual elimination of paper-based records. POC is fully integrated with Matrix to provide a manageable operational environment and security-protected real-time access to resident care information at any level of the organization.
POC can be accessed on a number of portable devices, such as tablets, kiosk PCs, and more. CNAs and other users can access resident care needs and assignment information as well as complete documentation on MDS supporting data and vital signs, intakes, and outputs. Real-time integration with Matrix coordinates the scheduling of MDS required charting and resident alerts for vitals that are out of range. This integration automatically updates the resident record with vitals details and provides the MDS supporting data at the point of MDS completion.
Fragmented charts prevent interdisciplinary sharing of vital information, which is why an integrated system is the key to a successful clinical operation – it provides real-time data sharing and communication that eliminates the need to sync systems or download information.
Point of Care helps long-term care facilities focus on their number one business goal - providing quality resident care.
Consistent and accurate medical record documentation – Documentation is a one-step process, eliminating the need to keep fragmented paper-based medical records.
Improved communication – Access to real-time resident care information is available to the entire interdisciplinary team.
Increased time spent with residents – Less time spent tending to paperwork and administrative tasks increases time spent with residents and providing quality resident care.
Precise care plans – POC documentation enhances the evaluation of care plan approaches and resident goals.
Improved staff management – Caregivers receive clear communication of resident needs and the tools to organize their time, documentation, and data. Documentation is organized by Matrix assessments and events and other work flow. The data that POC users access helps them in the decision-making process.
The Point of Care Look Back option allows direct care staff to see a defined amount of history regarding items documented in Point of Care to further eliminate the need for clip boards or other binders traditionally used to track toileting, vitals, and other tasks. Security driven setup allows the facility or corporation to control the length of time the look back is set for. Look back functionality allows care givers to plan tasks, keep on schedule, and see if there has been an important change in the resident status.
Optimized RUGs reimbursement through more accurate MDS supporting documentation, including ADLs, therapies, mood, and behavior.
Reduction in administrative costs – Electronic charting eliminates time spent organizing, recoding, and searching for paper-based charts. In addition, you decrease the amount of paper, duplication of effort, and improve the ability to capture accurate measurements of resources used for resident care. |
